About Your Long Term Conditions Review

This letter is to let you know about some exciting changes to the way your annual diabetes appointments are being made, to help you be more involved and get more out of your appointment.

We’ve changed how we manage annual check-ups so you get all your tests done in a single visit:

  • One Appointment: All annual checks done at once to save you time.
  • Birth Month Timing: We will call you closer to your birth month to invite you in for your yearly check.

Experience tells us that this will reduce repeated wasted trips to the surgery for you and also help us all remember when you have had your annual check- up.

If you need to contact us more frequently after your annual check we will do this.

How things will work for your Annual checks

Annual checks –coordinated with the month of your birth….
You are invited to attend an initial appointment where you will have tests such as your blood pressure, blood tests, foot and breathing tests completed.
