About Your Long Term Conditions Review
This letter is to let you know about some exciting changes to the way your annual diabetes appointments are being made, to help you be more involved and get more out of your appointment.
We are making these changes so that you get all of your annual tests in one appointment, and to reduce duplication. We will coordinate this with your month of birth. This means that every year you will attend the appointment for your annual check in the month of your birth.
Experience tells us that this will reduce repeated wasted trips to the surgery for you and also help us all remember when you have had your annual check- up.
If you need to contact us more frequently after your annual check we will do this.
How things will work for your Annual checks
Annual checks –coordinated with the month of your birth….
You are invited to attend an initial appointment where you will have tests such as your blood pressure, blood tests, foot and breathing tests completed.
- Your appointment will take 20 minutes
- Please bring an early morning sample of urine in a white topped bottle.
Sharing information – this is new and in response to what we know matters to people with long term conditions…
Your test results and other information will be sent to you before your next appointment. This will give you the chance to think about what you want to ask and talk through during your next appointment. So that we can send them to you via email/text so please make sure we have these details on file and that they are accurate.
At your annual care and support planning appointment (via your attendance at the Group consultation)
At this appointment you will be able to discuss your concerns and what’s important to you. It will also help you plan what to do to manage your health over the coming year.
- This Group Consultation appointment will take 90 minutes and you will also be joined and-learn from the experience of other people with the same condition as you.
- Please REMEMBER to bring your results and questions to this appointment so that you can get the most out of the appointment.
